Waiting List Application

the application can only be completed if your child(ren) is enrolled at Fall Creek Elementary School in grades Kindergarten through 5th grade.

To be added on the Waiting List please submit the application below and email fallcreeksap@gmail.com and assistant.fcsap@gmail.com to verify you have successfully been added and to receive a placement number. 

If your child(ren) is at the top of the Waiting List and a space becomes available during the school year, you will be contacted through email with further instruction.
